Volume Damper becoming Access Door

I'm having a weird issue I can't quite figure out.  Ideally we would be drawing in the correct service but I guess we don't live in a ideal world.

So..... Using Revit 18.3 with MEP fabrication parts.  A duct run is drawing with a 4" service. In this example its just a couple straight joints with a volume damper in it.  I highlight the run and use the change service command.  I select a 2" service and click finish.  The run is then switched to 2" but.... my volume damper turns into a access door.  Its very strange.  The similar task works correctly in CADmep.  Access doors have different button codes then volume dampers.  I have no idea why this is happening.  I can exclude the access door from fill and change service, then the damper just stays 4" and is no longer connected to the duct. 

Any ideas?

This is a bit different but on the same topic of access doors.  Is there a magic button or something I'm missing to get access doors to place on transitions or sq to rd?

I just eye snap them on! There is not a setting and will not cut hole either.
When someone asks where you see yourself in 5 years... Buddy, I'm just trying to make it to Friday. :)
