Revit "smart" sleeve

Was curious if anyone was able to create a sleeve that would automatically adjust to the pipe size and correct slab thickness or wall? Also, is there a way to make it auto tag properly. For instance, what system it attaches to, what pipe size and slab thickness
So far, tags are mostly manual input for us. I created a decent workflow but if changes will come it is hard to catch them
If pipe would move with sleeve that would be great too.
How do you guys manage all this stuff?

You thinking family or ITM? 
I think a family would work with native Revit parts.  As far as a ITM, if you make a sleeve out of a hanger CID it might stay attached to pipe if you move it. 
Not sure about the slab thickness part.

I was thinking about family. We have a family for Hilti Sleeves but it does not work the way I was expecting

One of the guys here has it working in Dynamo.  Sorry, can't share that one though.
