HETO Tap adjusting location when elbow placed after HETO

Issue with placing elbows on a HETO.

I have a rectangular duct line. say 12x12. Place a HETO on the side 8" round. It places it fine. But then I go to add a gored elbow after the HETO. It moves the HETO back down into the duct. As if its taking the length of the coupling required between the heto and the elbow and shifting the duct back. Please see the attached snapshots. Any ideas? This is not an issue in CAD. Only in revit.
